We did follow all those rules. However, you start with 3 hollow men on the board, and after the first epidemic, you're adding 12 hollow men into the top of the deck. With a turn one or two epidemic, that means in the top of the deck there are more hollow men than city cards (12 vs 9-11). By the the time the infection rate was 3 we were putting out 2-3 hollow men a turn. We did move them around so we didn't lose population.
Shelters don't stop you putting out hollow men do they?
