EACH player may perform these actions once per turn.
Also one player can't use the same location twice. He can use two different locations if he has enough actions.
As for the Bonus question: Only one player is left behind. You can play otherwise, but at times it'll make game impossibble to win. It is better to play with one loser only :)
But...the rules state that the group of escaping players must "exclued at least one player". This implies you can leave any number behind!
